341 Documents Checklist
You must attend the 341 hearing which will be held by zoom, and you must submit the following documents, or else your hearing will be rescheduled if they don’t have certain documents 7 days prior to your meeting:
1. Picture ID, such as a driver’s license or passport.
2. Your original social security card. If you do not have your social security card, you may submit a W-2 issued from an employer. (Tax returns cannot be used to prove your Social Security Number).
3. All bank account statements from bank accounts, prepaid cards, brokerages etc. from the six months preceding the filing date.
4. 2 yrs of tax returns federal and state.
5. Last 6 months of paystubs.
